The eggplant can probably be avoided but it's very tricky. The real "fuck you" isn't the eggplant but the item at the end of the video: that is supposed to be a full meter refill item (which nullifies the entire eggplant thing, see this webm) but there is a small chance you'll get this giant nothingburger in its place thus guaranteeing a death if you did trigger the eggplant.
There are a few "fuck you" item drops like this in the game (at least 2 that I know of) which further tells me the YT "longplays" that don't die are bullshit because you don't just need skill and remembering the entire game, you also need luck.

It's a decent challenge, but the filter is the final boss. He marches around in a circle, and will insta-kill you if he hits you. You need to hit him with perfect consecutive hits 20 times in a row to beat him. If you are a split second late in hitting him, he automatically heals back to full health.

Excluding puzzle games like Lolo 3, Solomon's Key, Milon's Secret Castle, Wrecking Crew, etc, I would vote for the Golgo 13 games, that crappy Dragon's Lair port, or Deadly Towers. Best of luck to anyone trying to beat those games. Special mention to Who Framed Roger Rabbit just because of how bullshit the final boss fight is, and the pure amount of bullshit you need to go through just to get to that point in the first place.

The final boss in SRnR is more annoying than difficult. It's hard to kill, but doesn't really pose an especially large threat.

I've beaten Golgo 13: Top Secret Episode and Deadly Towers. Golgo is fairly tough in general. Deadly Towers is mostly hard to figure out how to navigate, since the dungeons are huge and loop at certain points. Legacy of the Wizard is similar in that the enemies aren't really a huge deal, but figuring out how to complete the game is very hard, and doing things in a sub-optimal order can make the last two bosses tricky to deal with.

Ikari Warriors will always be the hardest. It's nearly an hour long with extremely limited tries. Your average game lasts 30 seconds. Without cheating you need to progress through nearly an hour of that bullshit executing peak ability to scout out the next 30 seconds of gameplay ahead of wherever your current pb is. Easily over 100 hours of complete frustration to learn without abba cheats.

Me and my brother were playing Ikari Warriors once, I don't know if we triggered a glitch or a code we didn’t know but either way we stopped taking damage. But then we also weren't careful to keep getting ammo so it ended up that the enemies couldn't hit us but we couldn't kill them either.
