I don't think it's so much a regional thing - my impression is that actual elevator music was very common in the 1950-1970 range, and that it made a big impact then and solidified people's impression of it as a very well-known cliche before dying off, leaving us with only the untethered cliche. Like how I grew up knowing that disco music was a big stupid joke that you should look down on even though I never actually heard any significant amount of disco music.
