You can play the game and never interact with the dlc content (it's not needed at all) unless you go out of your way to play it. Truly, you really have to go out of your way to get a boat to even start (at least in sandbox).

I think the fief owning experience sucks because there's nothing to fucking do at your own castle, or anybody's castle. There are no specific NPCs to meet or get quests from at them. There's no attached villages to the manor or really anything that makes castles feel alive. There's nothing you can really do once you have one except pick up and drop troops and loot, and shove money into projects you don't really get to see beyond wall improvements.
