It looks like she grabbed the transformer with her palm on the end opposite to the plug which is totally fine if the socket isn't overly tough. The mistakes was not moving her palm up when she noticed that the socket was really stiff, but she didn't and kept pushing, slipped and turned the transformer into a lever that snapped the other pin.
