Because she's still young and going into menopause she will have to be on HRT to protect her heart and bonesthe former is a slightly safe procedure and the latter is more of a last resort tried everything else option because of the consequences it has. Menopause happens naturally as women age, but if the uterus and ovaries are removed it happens literally overnight so her body now doesn't have those hormones anymore so it's basically like being on life support for the rest of your life and why it's seen as a last resort option without estrogen being produced her bones can start to become brittle (osteoporosis) and she can get heart disease because the estrogen isn't there protecting the heart and blood vessels there's also a balancing act because endo feeds off estrogen and if she has endometriosis that means the tissue can start to grow back there's also the issue of her PMDD which can make her hyper-sensitive to synthetic hormones so being on HRT can make her feel just as bad as when she had her ovaries and uterus still balancing act of pills and patches and such that can make her feel normal again that can actually take years to get right
