Years ago I saw an image that was black and white, woodcutting style vibe, and had a big circular "portal" and inside the portal was nothing but jet black. It was framed with various things including some skeletons, one skeleton looking in, but beyond that, the void was the centerpiece of the image.

Attached is the closest thing I can find so far, but it definitely wasn't this, even though interestingly this has one skeleton looking in just like the image I saw.

It's driving me nuts and I can't seem to find the right search terms.

I think you're looking for this. It's the outer cover art for Watain's Lawless Darkness. They believe in anti-cosmic satanism, like Jon Nödtveidt of Dissection did.

Anyways, I also suspect that this may be what OP was looking for. It was an album artwork, like I suspected. There's actually a wikipedia page about it, which cites more information: https://en.wikipedia.org/wiki/Lawless_Darkness#Artwork_and_packaging

Specifically, it seems that there are multiple parts of the cover art, and that there was a hole in the center of the outer cover let you look at the vortex in an inner part.

Does anyone happen to know what the text/inscription flanking the hourglass says? Looks like Hebrew ויא but I don't recognize the script of the left side.

Two numbers common with the anticosmic crowd are 11 (11 angles of Chaos) and 218 (Current 218, Chaosophy, 2+1+8=11). The letters on the right side of the hourglass almost look like the Hebrew numeral for 11, except it has that extra ו character at the end. May as well be a wild guess on my part, honestly. Not sure what the left side might be, either.

You could reach out to the artist and see what he says. The wikipedia page for the album says the band discussed symbols and ancient lore with Bielak, for his creation of the artwork.
