He's literally two bodies fused together, but from a Buddhist point of view it makes perfect sense. At that time deformed, burned, sickly people were considered cursed, because it was widely believed that your physical deformations were a result of bad karma from a past life, and so you must have been a terrible person in a previous reincarnation and only Buddhist priests and nuns accepted to get close to you. When people of the Heian era saw Sukuna, they had an explanation for his looks, one rooted in Buddhist, and we know that Gege loves referencing that religion every few panels. By suddenly adding aliens, he's unraveling every decent reference he made.
