Because comically huge boobs are simply fetish fuel and if you're making a manga that isn't purely fetish fuel you're probably not going to give your characters massive boobs. Doesn't mean they can't be big but the guy in that image seems to want boobs the size of what's in his image, which are the fetish fuel size I'm talking about. Same reason most mangaka aren't going to make a futa character if the series isn't specifically targeting that fetish.
