For me it's whenever a non-Japanese character speaks perfect Japanese but then gets caught up on an extremely easy word. I remember the Finnish girl in Senran Banka speaks near perfectly but then struggles to understand how to say tatami.

Pitch accent always seems like some bullshit to me. Anime characters sound completely different from real life Japanese speakers and it seems to me that the reason why is that the voice acting in anime doesn't stick as much to the pitch accent as real life speaking does. So a Japanese speaker should be able to understand a foreigner not rigidly sticking to the pitch accent system.

The need to convey information to a Japanese speaking audience. The same reason 'Allo 'Allo had everyone speak English for its English audience while using exaggerated accents to convey which language the character was using.

It's a bit different in professional settings, though. If it's a person working they usually insist on speaking English to you out of excessive politeness, even if they're shit at it.

I've seen pretty candid videos of Japs speaking about pitch accent autism in foreigners and almost all of them say not to worry about it, since they'll still understand you. You'll still sound like a foreigner, but you'll never be anything other than a foreigner to them, anyway, so stop tearing your hair out over it and just have fun. They don't expect perfection out of you with speech, even in work settings.
