He's adopted to the Tohno family, and after an incident with the real heir (SHIKI, as he is commonly referred) he was brainwashed to think he's the real Tohno Shiki. He's originally a part of the Nanaya family, with the manifestation of his urge to kill monsters sometimes being referred to as Nanaya because that's what they do.In Melty Blood, the fighting game series, which is essentially a set of simulations, Nanaya Shiki is considered a separate character entirely, but he doesn't actually exist, it's just an ACTUAL manifestation of the Nanaya side.With Ryougi, she had a split personality, a male and a female side, but the male side "died" in a car accident.Because of mage bullshit, there's a third one too, within the body, but that shouldn't be able to come out (It does anyways).

A Vampire Mage who has beaten death by marking women with a spell that makes them give birth to children with encrypted Roa hard drives in their DNA. When Roa dies, his next body is already ready to take this soul. The 3 siblings were playing in the woods one day when Roa began to possess SHIKI. An oni being possessed by a vampire caused Shiki Nanaya to crash out.

They couldn't compete when a genuine monster product of the Tohno family named Kouma Kishima came one day and killed them all because his body was impossibly durable and strong, the most powerful product of the Tohno family by far. The only injury he suffered was losing an eye in a battle against the Tohno patriarch.
