The brains are just straight up different, the biology would definitely throw it off, and I'm not sure exactly how brain development works, but you might just straight up be unable to retain all your memories. Mind or consciousness isn't something separated from the body, it is something emergent from the unfathomable complexity of the system that your body is. Different body -> different system -> different mind. You could try to rearrange molecules of the body A in such way, that it would produce consciousness identical to body B while retaining outward appearance of the body A, but there's no guarantee that it would be possible or that it would remain stable.
Isekai scenarios are just "it's magic, I ain't gotta explain shit", trying to figure things out using science will inevitably lead to contradictions once you dive deep enough into the subject. This was actually a minor plot point of that one slime tamer isekai, MC was having difficulty being a stone faced rational adult and the gods told him that was because his body influenced his mind.
