They didn't have someones brain/soul he was related to they could cram inside of a unit until his sister died. Then they put him in a unit, but it was experimental or some shit and got usurped by an angel and shinji had to fight it and it went fucking nuts and almost killed the pilot while trying to consume the A2 drive or whatever it was called

It was implied because we knew she was in the hospital and suddenly he got called to be a pilot. We know that the other 2 pilots (rei is a unique case) were only able to synch because of their relationship, either emotional or genetic (probably both) with the "soul" that was inside of their respective units, being their respective mothers. Therefore, it's reasonable to assume that she died and they did they same thing with her that they did with yui and asuka's mom
