The only problem with his physicality is that karate necessitates he keep himself in great condition to get the most out of it since it's a very direct and forceful martial art unlike Shibukawa's aikido. That said, don't forget that it's only been like a year since the MT. Doppo hasn't really had a chance to decline much, if at all, since he's still been actively fighting and improving the whole time. The loss of an eye didn't slow him down, the loss of a hand was just a minor setback, and nothing else has been as devastating as the thrashing he took from Yujiro back in the first arc.
