If you have basic instincts, and are able to think about the way in which you speak, then you have an internal monologue. Just because you are able to move and act without narrating every single one of your actions doesn't mean you don't have an internal monologue. There are complex thoughts and patterns that can't be identified with pure instinct, and the simplest, most intuitive way to describe them is as a series of words. Not having an internal monologue doesn't make you "cool". People who don't have an internal monologue are actual, non-verbal retards, the kind that act like babies.
