Oldest one I can think of that's not time travel is Hanakanmuri no Ryuu no Kuni (1981).
If you count time travel (I don't) then Drifting Classroom would be the oldest? (1972).
There's also Ryuu (1986) where the MC gets transported after getting into an accident.

Dunbine invented isekai.
Byston Well is referred to as isekai. The people there are referred to as Isekai-jin. The marketing throws around the word isekai liberally.
There's of course prior art of people going to other worlds, times, portal fiction type stuff in Japanese fiction. But Dunbine invents isekai as you understand the term.

Kinda funny how many things hailed today as a "deconstruction of isekai" were present in Dunbine.

It should have happened in parts the author was alive for.

Konosuba came out around the same time and was already making fun of common isekai tropes, no idea why Pedoshoku Tenshit fans push this narrative

Facts.
Mushoku Tensei is just head and shoulders above everything else anon, nothing quite like it has been written really.
There existed some very mediocre isekai series in the past, series that didn’t even manage to surpass MT’s mediocre arcs.
You see when it comes to isekai there was first the dark age.
Twelve kingdoms, escaflowne etc.
These are all works that were mediocre to the core, absolutely unremarkable series made for the female audience forgotten in like a few years and nobody even talked about them for ages. Do you notice how you have to constantly barge in MT threads and remind MTchads how their inferior series also exist? This is because they’re inferior and if you made a thread about them they would die quite fast. In fact they wouldn’t even get like 2 replies. I know this is a hard pill to swallow.

But anyhow, before Mushoku Tensei there was the dark age of isekai. Works such as I mentioned above were frankly trash and nobody took them seriously.
Then came the age of shousetuka ni narou: leadale no daichi nite, Meikyuu harem, Re monster and some new narou series started sprouting but also not many people gave a rats ass about those. Because they had this one gimmick and bam author wrote themselves into a corner they quickly devolved into cheap fast food type shlock. Nobody even cared how they would end and most of them never did end either or ended in a way that was awful.

But then came the light of civilization so to say. Came MT which was like the Roman Empire civilizing all the barbarian savages. It opened the gate. It opened the way.
Sadly of course, even after it showed others the way none of them managed to surpass MT which stayed as a masterpiece to this day.
