In Johan's mind that would have fulfilled the ending to the story, and thus given his own birth, life, near-death, survival, and final death a meaning and value beyond just being 'the spare/decoy' of his sister for their mother. It would have made his existance not just a series of unfortunate tragedies but a purppseful narrative. He would be the sacrifice in the creation of a true monster that could no longer be called human. And maybe some part of him believed he would live on in Tenma's fractured psyche after making him break his hippocratic oath and turn his monstrous kikdness into a tool for killing
