So Kawahara went to write a short story they could use, but ended up writing more of a novella that was way more than they needed. That also kick started him into writing Progressive. She was originally just supposed to be an NPC ally or enemy depending on your choices, then die at the end of the fight. Kirito and Asuna being way overleveled due to the whole deathgame thing meant she survived instead. NPCs in Aincrad had varying levels of AI to them, with most being heavily scripted with AI mostly limited to getting back on script. I believe Kizmel was one of the higher tiers of AI, and has been left running way longer than intended. Cardinal patched the fight with a different dark elf, made the death trigger no matter what, and provided a more generic NPC ally for other people doing the quest; but it's stuck keeping Kizmel online and updated for Kirito and Asuna. The result is she's a top down AI that's increasingly developing beyond her original parameters.
