Apparently the entire stereotypical scene (girl running with toast in her mouth crashes into a boy around a corner) never happened in its entirety in any manga in a serious way. At least as far as anyone can tell. But some manga did do parts of it (running with toast, crashing into a boy while going to school, etc). And iirc people joking about it has been a thing as far back as the 70s.

A likely candidate for the origin of the meme and the first manga to make that joke has been identified. Like you said, it's a composite of different parts. Different parts have appeared in shoujo manga going back since the 70s, and there are even shoujo manga with the full "girl running late to school crashes into a boy and they fall in love" scene but without the bread. There are manga characters who are shown running with bread in their mouths, so it's possible that these two things got conflated. The first known manga to depict the full scene is Saru demo Kakeru Manga Kyoushitsu in 1990, presenting it as a common encounter in shoujo, and from there, the idea spread.
