The twins were considered mistakes by the Zen'in clan because they were born so weak and Maki was born with a heavenly restriction. In that clan the strength of your cursed technique determines your social standing.

So after the clan head Naobito died of his injuries sustained while fighting Jogo, his son Naoya was selected as the next clan head, instead his younger brother Ogi, the twins' father. Ogi was pissed because while his cursed technique was up to par, it's because Maki and Mai were weak why Naoya was chosen instead of him. So he defeated them and threw them in a training room filled with cursed spirits, telling them to get stronger or die, it didn't matter which.

Maki had no energy left to fight back, so Mai gave her life to conjure the strongest weapon she could: a perfect replica of the Split-Soul Katana, Toji's strongest cursed tool that was lost after his death. Due to jujutsu sorcery rules, twins are considered to share a soul, and thus can only achieve full potential when they don't share energy anymore due to the death of one of them. Mai's death fully restored Maki's energy and awakened her to her full potential as a heavenly restricted, on par with Toji.

Maki then used the katana that Mai used her life to give her to butcher Ogi, Naoya, and anyone else in the Zen'in clan that created or supported the conditions that lead to Mai'sd death, which is just about all of them.

In the intervening years Maki came to regret Mai's actions, as despite it giving her the strength to fight back, after all the fighting was over her sister was still gone and she still desperately missed her, and wished there could've been another way.
