Because the author either has an NTR fetish he thinly tries to insert at times or uses it as a crutch for storytelling drama. One of his stories after the Nana series ended was literally just an NTR plot. It starts off the first 6-9 or so chapters being a cute-lewd story about this hot chick (equivalant to Nana's status) at school intentionally "fleshing" (more like giving lewd expressions or showing parts of her body) to the MC while at school. Then, as the story is still just starting off, it's revealed she has this weird incest relationship with her brother. The entire story from that point onward is the MC getting NTR'd and teased by the brother.

Forgot to mention that the whole "there's a better bdsm guy than me and he's taking my girl" you mentioned happens at least twice in the Nana story. I know the NTR scare was the big climax for the first Nana story, but something similar also happened in one of the two Nana spin-off/sequel stories with the two. I think it involved some elderly man, whose later revealed to have his own young bondage fetish girl. I believe that was in Nana to Kaoru: Black Label, but I never looked into the Pink Pure or Last Year sequels.

A slightly more detailed explanation from 287707784: the first Nana manga has a plot point involving blackmail and NTR, where a blonde guy around the same age as the MC gets involved with Nana. But it feels like cheating because she gives similar turned-on faces in response to this guy's BDSM antics, and some of this happens before the blackmail and rape actually start. This plot line was also in the last arc, which people didn't like. Black Label is a spin-off sequel to the original manga. One of the main focuses is on the two meeting a new character named Sarashina-sensei, who is a famous BDSM author. Nana idolises this guy because she's a fan of his work. He also has a hot blonde woman who is his consensual BDSM slave. He's not a bad guy, but since he's a fetish author and Nana is a fan of his work, it all feels cuck-ish.

He's some examples in pic related. As mentioned above, he's not a bad guy; he just wants others to learn from and enjoy his fetish. However, since the MC is somewhat passive and not very forward with Nana, Sarashina-sensei takes a hands-on approach to teaching them new forms of play and suggests ways for the MC to act. He really doesn't have any ulterior motives... but just look at these panels. He feels Nana up multiple times throughout the story, and she's clearly enjoying it. The entire premise of the Black Route is based on the idea that the reader connects the dots and realizes that Nana idolizes this porn writer... and then she's all turned on every time he feels her up, just feels like NTR.

It's why his stories fell off after the original. He started introducing NTR/cheating vibes everywhere and can't help himself not doing it.
