I haven't read a lot martial arts manga (because I'm not really into watching MMA irl) but I really like All Rounder Meguru. What are some martial arts manga that you like? I've tried Holyland but I don't quite like it, same with Tsuyoshi.

By the way I came across Tough (an early 2000s manga), through mangaupdate recommendation, apparently it was a sequel to 90s manga, but MU says you don't need to read the first series to understand it. I enjoyed Karate Minoru, but don't know if it would satisfy your need of realism.
I actually quite liked and recommend Karate Shoukoushi Kohinata Minoru, both as a somewhat serious karate shonen at times and a ridiculous coming-of-age teen comedy the rest of the time.And hey, no matter what, not many sports manga have moments like a ninja LARPer actually breathing fire in an official match like what the fuck dudeHell, even the sequel had it's moments before they dropped the ball HARD for the second half and atrocious ending
