I think it was deliberate and Kishimoto left a way for Itachi to come back. Just knowing the seals seems to let you resist the end of the jutsu and reenter the body until you can do the seals yourself. The problem is that Itachi was shown to vanish, but on the other hand his Izanami jutsu remained active, so perhaps his chakra stuck around inside Kabuto and he could have reemerged once everybody left and cancelled the jutsu on his end.
