Sadly actual Thai spicy and the secret menu with the actual good Thai food rather than the thai government sanctioned one all Thai restaurants are forced to have (real, look it up) is gatekept and you have to learn Thai to unlock it.
Honestly just be polite and say that they made it mild and you want more heat. I've never worked with or met a cook who doesn't get a little enjoyment over being able to make a dish spicier. Although some places are really just using pepper flakes and shit to increase heat. Find a places that use fresh thai/bird chili and you'll be set. Call and ask if you care enough.

I remember crying while sitting on some steps and eating while waiting for my ride in the brutal cold, soaking wet from the dish pit and thinking “yes, finally. This is what I’ve always been asking for.”
When I quit I made sure I left on good terms, and now I have a Thai place in town where I can get legitimately spicy food, it’s fucking awesome.
It’s been more than a decade and they still remember me :,)

another notable story about their cheapness was this one day when my friend (who got me the job) complained that he was hungry while we were working.
The owner then produced a folded up movie theater popcorn bag that she just carried around ON HER PERSON AT ALL TIMES I GUESS, gave it to him and said “free refills. do not lose. you never come work hungry again.”

It's just shitty gatekeeping which is shitty customer service the end of the day. That's just bad cooking. I ended up just telling people no refunds for Thai spicy orders and never had problems after.

I did give people milk for free if they ordered Thai spicy and ended up visibly near death from the heat though. Props for trying, better luck next time.

It was/ is an authentic Thai restaurant, and I was a white kid that just got kicked out of high school (despite having a 3.8 GPA, I was a real pain in the ass).
It was literally my first actual job.
Of course they wouldn’t let me cook, they took great pride in their food.
Even if they did, I’d imagine even the slightest fuckup would’ve cost me my job because it lost them money.
Her and her husband kept trying to get me to work FOH as a server because I’m apparently “very handsome it a waste for you to be back here, good for business out there,” but I was too nervous.

Looking back on it, I really should’ve taken them up on the FOH training.
I would’ve made way more money and they would’ve put up with me screwing up because they so desperately wanted me out front, I could’ve used that training to get a better job elsewhere.
That dish job started a long and storied career of BOH work (I did eventually work my way up to sous for a really great head chef elsewhere) that was ultimately thankless and fucked up my back something gnarly.
Despite being very personable and confident, for some reason FOH work makes me nervous as shit especially now that I’m pushing 30.
I feel like it’s too late to start.
But like, waiting tables part time at a nice place while working on my degree doesn’t sound half bad.
There’s a lot less responsibility and it seems as though you can bounce around with little to no guilt.
When I work BOH I feel a legitimate responsibility to both my staff and my work and it just absorbs my fucking life.
Part of the reason I’ve tried so hard to get away from it after all these years.

OP, no one has addressed a differrent variable yet. What is your appearance? Are you getting great service and food satisfaction? There are subtle and passive aggressive ways that restaurants have to keep customers or ensure they probably don't come back. If this is not a variable, the next time you go, make sure you are ordering something that should be hot, and then say that last time you were there, you could have enjoyed it more if it were even hotter, and then ask for a small side of sliced chilies to use as your own garnish.
