At least for me the whole thing is strongly shame related and genitals related, and I don't think at this point that you can get such a solidly engrained genital response if there wasn't a significant, probably deliberate genitals stimulation at some point (I think the shame aspect of the kink only adds to the probably traumatic aspect of its emergence).
