Paramedics won't even notice your clothes, they usually just cut them off with scissors. Even if you tattoo the words into your chest they'll just follow whatever the protocol is in your country

Paramedics (at least in US) are instructed to ignore anything like that.
They are not doctors. Barring specific, obvious, scenarios, they aren't even allowed to recognize someone as "dead" because that job is reserved for the doctor at the hospital.
That doctor is also the one who will have access to your file, which will contain the official DNR form, which he would use to make that call.
