Sister has an exercise bike like this and the pedals seem to have seized up.

Any ideas on the cause and fix?

The pedals or the pedal arm? If its just the pedals use some wd40 on them. If it's the pedal arm, has she ever checked the oil level on the motor?

Not the foot holders. More that the pedal arms aren't moving at all when you try to pedal

Pull the cover and take a look. Those are belt driven and simple inside so you should be able to see what's wrong. Are they very hard to move like the resistance is turned up or do they not move at all?
