Literally just put on muscle at this point and you'll fix ~70% of your problem. Not sure what your side profile looks like, but from the front adding to your traps, delts, and chest will produce a better presentation. Loose skin won't be resolved entirely by adding lean mass, but it'll be better than what you're working with now before surgery (if you go that route). Your situation isn't optimal but it's far from the worst we've seen here. Good job anon.
