How do these cable testers measure length? Do they measure the time the signal travels or do they use the resistance to calculate length?

Cable testers are basically time-domain reflectometers (TDR). It sends a signal and measures the time of reflection.

Reflection time shift. Only way to do it if you only have one side of a transmission line plugged in. If you had two sides you could factor in loss but then you'd have to know the cable impedance. Although, I guess you can probably assume a certain impedance if you know it will be a cat 6 cable.
