Where do you start to make this sort of distortion? I understand how to place straight 3d boxes in 5 point perspective but how do you go about a bunch of curved 3d forms and body parts rotated in space randomly like in this picture?

That's probably why he drew them, so he could just eyeball the perspective.As you can (or should be able to) see, they don't line up perfectly. He messed the perspective up a bit, so applying grids or whatever are just going to be confusing and frustrating. He eyeballed the perspective, got it a bit wrong, but not wrong enough to look off. If you're trying to eyeball perspective, remember that all perspective is "object further away = smaller", so no matter what if you can identify a solid guide in an image for objects disappearing into the distances (such as parallel lines) then you can eyeball the perspective.
