There is no canonical answer to what Miko was as Prince Shotoku, whether she was a man and became a woman as part of her reincarnation in the Shikaisen ritual, or if she was always a woman and similarly to say, Saber from fate/Stay Night, either hid it from the public or was recorded as a man by historians.This is most likely a deliberate choice on the part of ZUN so that fans could choose which option they would most prefer. Like many lore questions about his characters he gives no answer for.You may believe whatever you want about Miko but do not ever push it as textual canon. Instead focus on simply appreciating Miko as she currently is.

But I am actually really fond of her two hair bits, it is a very unique hairstyle that looks wonderful;2. Her earmuffs are another design element I really like. It is unique among Touhou characters and helps reinforce the modern element of her character;3. I like her outfit. Specially how it is seemingly plain but has just the right amount of little elements and accessories to help accentuate the design and provide a more final boss-esque appearance. From the gold anklets and bracelets,to how ornate her sword sheath is compared to that of say, Youmu, to the black transparent cloth under her skirt. And of course, the cape she has in the fighting games is a great addition, truly making her design feel complete;4. I really like her theme song. It conveys a sense of casual power, like you are facing someone who is not even trying to overwhelm you but just does so naturally, which is fitting for a character like her. And I also like how the theme is composed in such a way that one specific section will often play during one specific Spell Card for most players;5. Her personality, much like the previous elements I spoke about. There is a sense where you would expect her to be more bombastic and in-your-face than she actually is. While she certainly has her loud boasting moments, she is a surprisingly subdued character from what one would expect at first impression, and I really like that. I also really like how she has her convictions but is also open-minded and accepting of that which is contrary to her current beliefs if that manages prove itself valid to her, which I find to be an admirable trait;6. Adding to the above, and going into fandom elements. I appreciate how Miko can be portrayed in many ways. You can portray her as cool, as cute, as annoying, or even silly or sexy. And none of it ever feels particularly out of character or unfitting. And it is not as if this comes from her being a blank slate, it is just that her personality in official material as is is already shown to be very flexible and adaptable;7. I find her relationship with other characters fascinating in how she very clearly puts herself and her own interests above everyone else yet is still willing to do actions for the benefit of others. Not out of a sense of machiavellian manipulation (Though she is perfectly capable of that too still) but simply out of recognition that it does not go against her own well-being to do so;8. I find her ability to be interesting. In general I am very fond of the character abilities that are less of a massive strength or power and more of a thematic trait that informs their character;9. Her lore and backstory is fascinating. I would very much recommend anyone interested in reading up on the Nihon Shoki and the records of the history of Prince Shotoku, it is all very interesting. Which is also why I really like when things linked to him like characters such as Saki and Okina are introduced;10. Ten Desires was the first Touhou game I managed to 1cc, so I have to a degree a personal attachment to it and Miko is a big part of that original experience.
