There is no canonical answer to what Miko was as Prince Shotoku, whether she was a man and became a woman as part of her reincarnation in the Shikaisen ritual, or if she was always a woman and similarly to say, Saber from fate/Stay Night, either hid it from the public or was recorded as a man by historians.This is most likely a deliberate choice on the part of ZUN so that fans could choose which option they would most prefer. Like many lore questions about his characters he gives no answer for.You may believe whatever you want about Miko but do not ever push it as textual canon.
