In all my years of handling, cleaning, and sharpening various blades the only time I've ever cut myself by any serious degree was when I was washing my chef's knife while drunk. The sponge started to slip out of my hand as I was scrubbing the blade, and I instinctively curled my fingers to try and grip the sponge and sliced into the tip of my right middle finger. I have a little numb spot at the tip of that finger now. I use these long handled dish brushes now for washing knives so my hand stays far away from the blade. Not handling knives while drunk is also a good idea, but fuck that.

Lol, I've practiced eskrima for 14 years, an instructor for 8, and regular spar and handle knives. However, the only time I was injured by one was when I was washing a chefs knife and it slipped from my wet hands. Fell perfectly vertical, and pinned my bare foot to the wooden floor. Fortunately, it missed the dorsalis pedis artery.

I've become much more cautious and deliberate while washing my knives after that incident even though it was just a relatively minor cut that didn't require stitches. It's easy to become complacent when washing your kitchen knives, because you're not actually intending to cut anything with them, but it's actually much more dangerous since you're handling them while they're wet and slippery. Something else I've noticed I need to work on is being aware of others around me while working with a knife. While I'm using a knife for cutting I am conscious of where all my body parts are in relation to the blade, but I also tend to become very focused on that specific task as a result. I've noticed a few times where someone will be standing next to me, and I'll move the knife towards them without meaning to. Of course I've never cut someone, but I nonetheless get this sinking feeling when I realize how close I could have come to inadvertently hurting them.
