Why don't the doctors whose one job it is to treat us know anything about how to treat us?
I just found out Gabapentin, the 10th most prescribed drug in the US, makes your body basically unable to absorb estrogen. It significantly decreases "free estrogen", which nobody tests for so your "total estrogen" in the Planned Parenthood lab results might show great levels, but it doesn't mean anything since your body might not even be using any of it, meaning you've been masculinizing the entire time. These studies are from years ago, so it's not any thing new, and I mention Gaba every time they inquire about what medicines I take, so do they not keep up?
And it's not just that. How come trans clinics in the US still only use Spironolactone and oral Estradiol instead of the better AAs every other country uses, or even addons like Pioglitazone- perfectly legal to prescribe and honestly even less risky than 200MG of Spiro is? Why don't they know anything about how Progesterone works, how to administer it, its effects on DHT or its reduction of Estradiol? During my most recent visit I mentioned being concerned about Estrone from oral Estradiol competing for receptor saturation when they said there's no difference between oral and sublingual pills- they just stared at me.

Because spiro works most of the time and it's an old and well understood medication that is perfectly safe to take for a prolonged period which is the case for troonies

Here's what you seemingly dont get: suppressing testosterone is far more important that estrogen levels. I've seen BEAUTIFUL trans women who got amazing feminization on 50mg spiro every 24 hours and 2mg birth control pills. They were averaging 40pg/ml E and feminized beautifully.
YOU SPERGS DON'T NEED TO WORRY ABOUT E LEVELS AS MUCH AS YOU THINK. T suppression is the most important factor.

Anecdotally this lines up with what I've witnessed. If you're going to take Spiro, you probably should just take 50MG and merely enough estrogen to take your T levels down to cis levels. But having T at 0 is really bad for you too, which doctors rarely warn about.

Completely anecdotally nonsense. I took spiro and oral E for 10 months and I got healthy breast growth, sex drive was still there and I'm the kinda person who needs to sit before getting up otherwise my vision goes dark and I wasn't collapsing on spiro. I was on 100mg mind you. The diuretic effects are far more annoying.
Spiro works and it's safe to use it long term, that's why it is justifiedly so the first option of endos.

not that guy but we have one of these in my small town and i stopped patronizing it because of said political sloganeering. Most other people here did the same and it is now out of business, the building was bought, demolished and now several stories of affordable housing are being built on that site

Doctors are often trained very poorly on gender affirming care. They dont understand the medication very well and all they have to go off of is some antiquated study conducted in the 1990s in Canada with a very small test group. Hence why they end up hon dosing everyone and prescribing things like spironolactone. Although insurance coverage is partly to blame because they wont cover anything not involved in the previously mentioned study from our friends up north.

I had a problem with my previous doctor when I switched from taking estradiol pills to injections. She had me taking 5mg estradiol valerate every 2 weeks. (despite being on hormones for 2 years) Which put me in a state of detransition. (the heat flashes were nightmare-ish) When I told her I needed a larger dose and injections every 5 to 7 days instead of every 2 weeks, she tried to put me on 2.5mg every week and I chewed her out. I had to twist her arm and threaten to go back to the DIY market just to get a realistic dose of EV along with 200mg progrsterone.

Those blood clots are extremly rare and doctors are just concerned about you detransitioning and suing them. Resulting in the loss of their medical license.

yah my kino perscriber was really adamant on finding the dose where id have t levels at like 10 or so 7 days after my injection which is pretty low but also not too high that my t wasnt being suppressed, i wanna get an orchi and get on a lower dose eventually though, not sure if id just have 0 testosterone then i heard youre supposed to have a teensy bit

Could be, Gabapentin is prescribed a lot for anxiety. OP's study was only done on cis women but looking around, anecdotally, Gabapentin seems to mess with menstrual cycles often judging from forum posts. And it's often prescribed for relief from menopause symptoms, so there's clearly some binding action going on..?

I'm not certain whether the effect would be lessened by just taking more exogenous estrogen, as MtFs do anyway, since the study was only done on endogenous estrogen (and who knows at what Gabapentin dose), or what the mechanism at play reducing free estrogen even is, but there's certainly some connection. Although little actual research has been done it looks like.

But it does say "significantly".. So it could be harming a lot of transitions yeah. That's just my guess though.
