It looks like he reached a certain worldview in which he saw everyone part of the same being, even though split into individuals.

Everyone being gay means everyone being attracted to themselves, sharing their endorphins and feeding off each other, as part of that same being.

Whereas the hetero thing is the contrary dynamic, that makes people get out of themselves and project their attraction on the complete opposite of themselves in every way (body form and psyche).

I think people oscillate between these dynamics and are conflicted about them. So Cobain putting that in a lyric was about creating cultural shock.

The idea is not that complicated, it's similar to Spinoza's pantheism.
Tho there might be some Eastern philosophies who also share this 'great chain of being' view in a different form.
Nietzsche also reached such a worldview during his 'letters of madness' when he said 'I am every name in history'.
Could be a kind of vision that few get to reach and only at the end of a long process of understanding yourself to the point you've understood others as yourself too.
Maybe that's what that cryptic advice 'know thyself' from the ancient Greek temple was hinting at.

You can be sure when he says something that seems simple that it's likely part of a web of meanings that are much deeper and hidden behind double meanings
So he was definitely not saying just that everyone is gay, but this tied with a more general view on being human, like that in 'Milk it'

Though occasionally this obfuscation game he was playing, making the song lyrics appear to be about something deep, was just a mask. Drain You for example is about his love for heroin but it's couched in this image of embryos playing with each other in a womb
