I test drove a smoker's car earlier today (I didn't know it was until I was handed the keys and sat inside) and I can still smell the funk on my hands later into the night. Not surprisingly the car was advertised dirt cheap so that's why I went down to the dealer to take a look at it. Are there actually people out there that would tolerate a smoker's car for a few thousand less due at signing?
Get an Ozone Generator, retard. I've gotten the smell of decade long dead mice, smoke, and a suicide out of cars with one before.

Get compressed air and see if you can clean the tubing as well2,5. Go over the entire vinyl/pleater/leather/rubber interior with either makeup wipes or cleaning wipes3. Use the ozone generator for recommended period of time4. Go over the leather with conditioner for leather/pleater 5. Spray over the inside with dry vacuumable textile cleaner and vacuum again 6. Install new cabin filter 7. If it still smells by this point, its either the textile surface on roof/pillars, or embeded into the seats But since you vacuumed the interior before, and wiped down anything that could seal in the smell, you most likely got everything.
