I'm from a city that has a rivalry with Cologne, so take this with a grain of salt, but I will try to be as neutral as possible:
I don't really know what you mean with "bad", but it has gotten worse since the 90s for sure, though I am not old enough to judge more than the last 10-15 years. It's gotten dirtier and uglier, but it's still a decent city for the most part. The city has character at least, and it's not a criminal shithole. Chorweiler is probably the worst part of it, but you won't end up there anyway. So I don't think you have to be scared, it's still a safe city.
The only real major attraction is the cathedral, though. There, you will probably see the post-war misery of Cologne in general, as the destruction of the war has destroyed most of the historic town core. As a local, there are many other decent corners, but as a tourist, you experience a city with different eyes
Köln has gotten worse and Düsseldorf has gotten better since ~2010. Partially

So to clarify: the cathedral is worth a visit and so are the remains of the old town core. There's not that much else to see, but it's okay. And Bonn and Düsseldorf are right next to it, both can be worthwhile for a shorter trip as well. There are some nice castles/palaces and parks in the vicinity as well
Depends on how much time you have and how you can travel. But there's no need to be scared of Cologne at least

Oh, I forgot that since we're /sp/, visiting a home game of FC Köln would be another good thing to do when in Cologne. It's a nice experience (usually). But getting in as a tourist is almost impossible unless you plan ahead

Yes, that's the standard for basically any significant German club (Wolfsburg and the likes aren't significant in terms of fan culture, we have a big divide between "recently successful clubs" and "traditionally large clubs"). It definitely makes things much easier. Bayern's stadium has the inferior atmosphere according to general fan consensus because of the comparatively high amount of tourists and bandwagoners, but it will have the better football and surely be worthwhile anyway. Nice that you got tickets. So you will see them play against Bremen?
Oktoberfest is a scam in my eyes, the prices sound ridiculous. But then again, I am not a Bavarian, and I hope you'll have fun there. Bavaria itself is a nice area to visit, I can recommend Regensburg, Straubing, and Passau, perhaps Ingolstadt as well. These are somewhat close. But many small villages in the countryside will be neat as well, the area around the Starnberger See or the Tegernsee, for example. Or the Alps of course. Further up north in Franconia, there are Bamberg and Bayreuth.
If we're talking about nice cities across the entire country, I'd think of Dresden and Hamburg, Freiburg, Heidelberg, Erfurt, Lübeck, Münster, maybe Trier ... and the Upper Rhine Valley with its castles. I'm just spouting names here, though, check for yourself what you'd like and what you might be able to visit

Get a local state train from Cologne to Aachen, very much worth the day trip
Enjoy German trains and factor in being extremely late especially with changes. They are even worse than ours
