Almost always, residents of those smaller cities with low A, high A, AA and AAA teams are fans of the closest major league team. (For instance, I live in New England, and those who live in Manchester NH - home of the Fisher Cats, the Blue Jays’ affiliate - are fans of the Red Sox). The Major League just simply has too large a grasp on the baseball world. If 3 teams get regulated and 3 AAA teams get promoted, that would cause too large of a shakeup — do fans follow their hometown minor league team that’s now in the Bigs, or the team that their players get called up to, or the major league team that they’ve truly been rooting for all their life?
