Merge North and South America. Merge Asia and Oceania.

That gives you
>Americas: 51 teams
>Europe: 55 teams
>Asia: 60 teams
>Africa: 54 teams

Each region sends 6 automatic qualifiers and 12 playoff qualifiers. Playoff teams all go in a big pot, unseeded, randomly drawn, and play a two legged playoff.

Each WC the region with the lowest number of teams coming through the playoffs loses an automatic slot to the region with the highest number, tiebreaker being the regions performance in the actual tournament.

Perfectly fair on everyone, and should be self-balancing, as every region that loses an automatic slot is now sending better teams to the playoffs, and every region that gains one sending worse.
