What are some trolley problems in games you have been in?

In a Godbound game I am in at this very moment, part of the scenario's backdrop is that there was once an adventuring party of Godbound who went around the world, generally saving and uplifting it. All but one of them died for varying reasons, and of course, resurrecting a Godbound is virtually impossible.

Stricken with grief at losing his friends, the survivor went around the realm, gathering the patronage of multiple dragons, Eldritches, Fae, parasite gods, and Uncreated. By pooling together all of their mystical prowess, he forged a mystical contract: he would be drained of nearly all of his power as a Godbound, which would then go towards an evil tulpa-twin of his. This tulpa would track down the descendants of the deceased Godbound and murder them. (They were minor supernaturals, very hard to resurrect.) Then, and only then, would the survivor's friends all be resurrected in good health.

We gradually pieced these facts together. We ultimately voted on stopping this scheme, since most of the players thought that it would be disrespectful to the late Godbound. We used Brilliant Invention (Engineering lesser) to Burn the Bargain (Freedom lesser), eventually shattering the pact.

I found it to be a fascinating scenario.

The only time I had the party consider ethical side of things was with two questgivers asking for the exact same item, offering the exact same reward, and conveniently located within the exact same travel distance from where the item was ultimately discovered.

To be clear, the survivor's plan actually had considerable merit to it. These deceased Godbound were morally upright heroes who used their demigodly powers to save, reshape, and embetter the world: before their untimely deaths, anyway. Bringing them back would have, in theory, restored to the world a collection of messianic figures who could have continued to good-heartedly uplift the realm.

Destroy dwarves because they refuse to hand over a pedophile, or let a kidnapped elf girl die. The players were stonewalled by human nations refusing to choose a side over economic relations. To explain why I choose to display dwarves as evil - 'that guy' is almost without fail a dwarf lover and elf hater.

My rogue trader party had managed to upload a mad AI anime girl into the ship cogitator array. The ship Enginseer rectified this by physically overloading the plasma reactor, forcing the AI to choose between shutting fown shipboard power feed and transferring to icky servitor wetwares or becoming a cloud of radioactive space dust.
The AI chose the former, as a result the bridge crew went on to crawl the unpowered corridors to reach ship castle (both in the sense of a fortified military installation and a big stone building with walls and towers) where they met up with the arch-militant's second-in-command and his flamethrower maids who laid down the whole situation with the reactor to them and hooked them up with the trolley in question (Piloted by the local death cult leader and pulled by two giant crabs), which brings us to the trolley problem.
As the party is hurtling down the tram tracks of the ship arterial on the way to the plasma drive they had to choose to continue on the current track, which would make them run over a dozen or so servitor. The servitors were tainted at this point but the techpriestess (and future wife of the rogue trader) running the life support system is a shameless Xenarite and the ship will be taking serious penalties from losing almost all the servitors. However the other track had a pair of techno-heretical mechadendrite squids from the Matrix using corrupt-AI-psychic powers to mindcontrol a group of innocent voidsmen (usually mindcontrolled by the local slaaneshi cult addictive junk food and gambling dens). There was also a couple of big loading servitors standing around precariously close to a giant and unexplained pitfall in the middle of the ship.
The party put on that Japanese tram game soundtrack and multi-track drifted through. One crab died. We miss him forever.
