Basically vampires are created by worms that burrow under people's skin and transform them into mindless (and hairless and sexless) animals over the span of a few days. But these vamps are linked to a hive mind controlled by The Master (a grotesque ancient creature). And the Master selects a rare few of his creations to retain their minds, put on makeup to hide their transformations, and infiltrate society to do his will (and he knows they're doing his will!). And the story is about how a team of disease fighters try to understand and handle a rapidly growing outbreak of vampirism in modern New York. Some people like the books better than the show, but I only know the show, and it's -generally- good (until a plot thread involving a major McGuffin is effectively just dropped).
