What happens if two liches decide to use the same object as their phylactery? Has this ever happened in any settings lore? Does one get shunted out? Do they have to share? Do they fuse?

There are several reasons this question is stupid.
First is that the ritual to become a lich is individualized to each caster requiring unique reagents and items, so they wouldn't share.

The second is that casters who become liches are generally paranoid egotistical sociopathic loners and a second person trying to become a lich in close proximity is a danger, so doing it by yourself away from others is paramount.

You cannot enchant an already enchanted object and two casters trying to enchant an object at once cancels each other out. One or the other gets the object.

A phylactery is not any random object, but a purpose-built item decorated in jewels and precious metals within which is kept a series of spellbound scrolls that act as the receptacle for the lich's soul. Its individualized nature is a reflection of the caster's personality and soul and an important component of the ritual. A phylactery that is not built by the lich will not work. (and why /tg/s obsession with cosmic liches who use concepts and abstracts or immensely large objects like moons as phylacteries are retarded)

There are other reasons but these are the big ones that prevent it.

Seems like the simplest answer is that destroying the phylactery destroys them both. If the liches are gregarious enough to share a phylactery in the first place, they get the benefit of being able to split the production cost.

Says who? It was certainly possible in 2e and 3e, at least, since both of those had magic item crafting rules that often required existing magic items, or allowed for upgrading existing magic items.
