It's basically just a reflavored leadership feat made into a prestige class, getting a few extra telepathy powers thrown onto your "Powers known" list, a couple buffs to some specific powers, and a capstone of giving you a second companion (albeit at a lower level than the first). It's something that could easily be reworked as like a cult leader or similar without a ton of effort.

I think more DMs would be put off by the "goblin fucker" concept rather than the mechanics it entailed.

If the game is hardcore number-crunching battles where I'm expected to min-max off the bat with no wiggle room for levity, I probably wouldn't be enjoying myself anyway. I prefer more narrative-based stuff. At least when it comes to TTGs.
