However, he later continued to fever and was found to have significantly low blood pressure, along with altered mental status (was only able to answer yes/no questions, typically far more verbose per family). Because of this, he was transferred to the ICU for continuous artificial blood pressure support. His antibiotics for UTI were also escalated at this time as well. He was gradually weaned off of his blood pressure support and was transferred back down to the floor. However, he then started to fever again, had sudden drops in his oxygenation levels, and became significantly confused. Because of concern for his ability to support his own breathing and protect his airway, he was intubated and placed on a ventilator prior to being transferred again to the ICU. Investigation revealed pneumonia, which was thought to be hospital-acquired given his long stay for his initial UTI. Appropriate antibiotics were started for this. He was gradually weaned off of the ventilator and extubated. After monitoring for stability, he was transferred back down to the floor. Patient was monitored further on the floor, and remained improved. However, due to significant deconditioning from his baseline over the course of his hospital stay, he was seen by the Physical Therapy team and deemed to require further strengthening. After discussion with family and patient, they agreed for him to be discharge to a Skilled Nursing Facility for further continued intensive physical therapy.2/X
