The basic premise is that it's a horror game in a setting where looking outside has started causing you to mutate in horrific ways, usually merging with shit near you. One guy in your building merges with a pitcher plant and there's a unique gameover if you fight him alone, one guy merges with his cameras and can steal souls, and one guy in a very specific ending route can straight up oermanently devour your party members if they fall asleep during his fight.
