Obviously I think it is way more hot if the werewolf transformation is permanent.

Probably done outside of the public eye too. Thing is, I can imagine said werewolves being really possessive about the humans they are slowly taking into werewolves. They probably see it as one of the most intense loves in that setting to turn a puny human into one of them, so it's not like an entire pack is descending upon the human in one go. Or if certain packs are being really suspicious of recently former humans, especially if the societal gap towards humans really is that big in that setting. Your plot does also raise an interesting point, what if humans due to that medicine and general class matters do not actually believe it is possible for a human to turn into a werewolf? I can imagine the psyche of the human character in that story not taking that transformation as well as most of the people in this thread would, to put it lightly. Let alone said human having to adapt to his new body. It really is a setting you could take in a lot of interesting ways.
