Normalfag tier answer: The Human Centipede. Popular enough to get parodied in South Park, wasn't marketed as a straight horror movie.
Online normalfag tier answer: A Serbian Film. Not something people would discuss IRL, but still very notorious.
/tv/ tier answer: some shit like Bad Boy Bubby or something else that doesn't try too hard for no reason.

Of course there's stuff like August Underground, but let's be real, it's just people fucking around with a camera.

i watched most of Mickey 17 on the airplane and there were some parts that were mildly disturbing. Mostly because 17 slightly reminded me of some autistic guy i know

For me a disturbing movie is usually something quiet and unsettling that gradually builds dread and doesn't offer any relief or closure - I think you can't go wrong with The Seventh Continent. Felt like I was watching a family's home movies, and it stuck with me. Haneke's later films like Funny Games are more cinematic and over the top with their nihilism, but something about the stripped down, straight forward nature of The Seventh Continent nails that melancholic hopelessness for me
