Sleep apnea-induced hypoxia actually. She basically stopped breathing and couldn't receive immediate medical attention because the plane was still awaiting clearance to land. Now it's standard FAA procedure to give any aircraft with a medical emergency priority landing clearance (waving off other incoming aircraft, stopping all takeoffs, and having the plane land at the first available runway) but it still took 15 minutes to get the plane onto the ground and docked at the gate until paramedics could actually get to her. Irreparable brain damage occurs in as little as five minutes..

The mask wouldn't help because it's intended for when there's a breach in the cabin and all the oxygen has been sucked out due to loss of pressure. The problem with Fisher was that her lungs had stopped inhaling/exhaling and thus weren't pumping oxygen throughout her body and having CO2 rapidly build up in her blood as well.

Since we're discussing Star Wars, think about it in terms of Darth Vader's death in Return of the Jedi. There was plenty of oxygen on the Second Death Star for him to suck in, the problem was that the machine that controlled his artificial lungs had been short-circuited and after 20+ years of that thing essentially breathing for him, his actual lungs had become too weak to inhale enough oxygen. With each passing moment, he was becoming more and more hypoxic. He knew it was a mere matter of time before he lost consciousness and died, hence why he begs Luke to take his mask off so he can look on him with his own eyes.
