The twist was that eventually the gay men sacrificed sex with each other to save billions of people. In the realistic expectations of the viewer, the gay couple would not choose to forgo any future sex with each other by one of them killing the other, no matter how severe the consequences for the rest of humanity.
Furthermore, there was the initial big twist at the start when it was revealed that the gay couple had chosen to adopt a little girl and not a little boy, and the real headfucker when you realise that they probably haven't molested this infant, as you'd expect of a gay couple.
