I was just watching the commentary track. I agree with RedLetterMedia. He had a schizoid embolism during the implantation. His mind couldn't handle reality anymore. That's how real the dream was.

His mind died at the Recall facility. As far as his brain is concerned, he saved Mars and got the girl. Quaid didn't exist anymore.

What if that's all a part of the dream story?
Like everyone that does "Blue Skies over Mars" goes through the same 'Is this real?' process to help enhance the believability of the Rekall dream.
Personally I think the whole movie from start to finish is the dream as Quaid was implanted before the movie started and we're witnessing the actual memory play out (even with Quaid thinking he looks like Arnold Schwarzenegger and unhappily married to a VERY comely Sharon Stone).
