Everything about his POV is disturbing for a young adult book, or at least it was to me, at one point he reminisces over smothering his baby brother because he was being too loud in the crib. It's not so much the dissection of his psychopathy that is disturbing but how it is written, there's more of the usual stuff like slow-killing puppies and such. The fridge thing i think is that he put the animals he gutted in the fridge and then checked on it once in a while to see if they were dead yet, there might also be a moment in that chapter where he forces another of the bully crew to blow him but I might be imagining that. Anyways, when I read the book as a child, I found his chapter waaaaay worse than the sewer train, the exact social mechanics of sex weren't as clear so I just figured it happened that way sometimes and blew past it to get to more interesting stuff.

Also, the longer you walk, the more lean you become, lightening the walking load, making longer distances easier.
